New patients can use our secure Patient Portal to send us their Medical History Forms.  Please call our office to set up your Patient Portal. Patients may also read consents for treatment, pre-treatment and post-operative instructions in preparation for dental treatment. Please find the link for the forms below. You may also print out the forms and bring to the office on your first visit.
